We believe everyone deserves access to healthy, nutritious food — especially when managing a health condition or major life change. Our program works with Medicaid to deliver up to 3 meals a day, right to your home, completely free of charge.

WHO QUALIFIES
You May Be Eligible If You:
If you have Medicaid Managed Care and meet at least one of the following criteria to qualify.
Are pregnant or recently gave birth
Have a child under 18 with a chronic medical condition
Have a diagnosis of mental illness
Have a diagnosis of substance use disorder
Are enrolled in Health Home Care Management
Medicaid high utilizer
